All LLCs formed in Colorado must file the Periodic Report. The filing must be done within a 5 months window, it can be filed as early as 2 months before the month of anniversary of creation. The due date for the filing is 2 months after the anniversary month. That is, it can be done between 2 months before and 2 months after the anniversary month (5 month window).

The periodic report is due within a 5-month window: 2 months before the anniversary month, the anniversary month, and 2 months after the anniversary month. The filing should be done within this 5 months window.
Failure to pay
If the Periodic Report is not filed on time, the entity will have an additional two months after the Periodic Report month to file a late Periodic Report. Colorado charges a $50 penalty if you miss the annual report deadline. On top of that, if you fail to file your annual report for 2 months, it will cause your LLC to be designated as "Delinquent," resulting in loss of certain rights and privileges.
